Three voices in one hand (staff)

I'm having trouble getting LilyPond to do what I want.  I have three
voices in one hand (staff).  I want them to merge and line up, but I
can only get one or the other.

I've attached a picture of the output.  Any ideas on how to merge
without a shift?
