Honestly, I think the best would be to work towards reducing or completely 
eliminating the \paper/\layout distinction. For me at least, it's 
incomprehensible. I mean, generally speaking, \layout affects only music while 
\paper affects paper, page layout and music, but the implementation is muddled 
enough that I cannot even tell if this rule is absolute or not. (For example, I 
don't remember off the top what we do with \markup \score.)
