I care about this complicated problem and I've worked a lot on it. I've made progress taking approaches where the presence of time/clef/key grobs triggers the engraving of separate end-repeat and start-repeat bar lines, and where clef/time/key changes within \alternative are understood as warnings or reminders and are automatically moved relative to the bar lines. The central difficulty is making everything work together without circular dependencies.
I gave up trying to achieve any of this for 2.26 and decided to try to tie up a bunch of work on meter and timing. I still care about it, though.
