BSD crowds mightn't need such a thing, but Windows and Mac users might

I'll definitely add a note to the docs that midi2ly is a separate
program, to be run on the command line. I might add some links to
tutorials on basic command-line use, since OSX and window users don't
know how to do that kind of thing. (which, IMO, is a truly sad thing
-- how can you interact meaningfully without a keyboard? :)
I'm not going to try to explain how to use a terminal directly, though.
There are a million basic terminal usage documents on the web; there's
no point in adding one more. (well, three more :)
Another option is to add midi2ly and xml2ly (but probably not the other
ones) to the lilypond app menus. If any windows/osx user would like to
sponsor that, I'm sure it would be useful to lots of new users. :)
