Myriad (the makers of Harmony Assistant) have a program called PDF- to-Music that does pretty well, but then you're stuck with the file in their closed-source format.
They've also OMeR as "standalone" music OCR, but it saves only in Myriad's MUS format (used by Melody and Harmony Assistant). When I tried it, the music input was usable, but it couldn't import lyrics. And I'm rather sure it wouldn't reckognize handwriting.
Harmony Assistant can export to MusicXML. I never tested that, though.
