Hi Michael,
the trick for having repeats in MIDI is documented here:
http://lilypond.org/doc/v2.11/Documentation/user/lilypond/Repeats-and-MIDI.html
Basically, you have to copy your \score block, add a \unfoldRepeats
block around your music and a \midi { } block.
