I think your specific readability criteria should determine whether
you combine noteheads or change the stem direction
i.e. you shouldn't normally let a Lilypond collision force you to
abandon your choice of presentation.
There is something funny going on here.
It looks like the collision avoidance is positioning the second note
as if the first notehead was the same size as the second notehead.
Putting
\once \override NoteHead #'X-extent = #'(0 . 1.3)
before the a8 (_not_ the a2)
makes Lilypond think the second note isn't smaller: no collision.
The subsequent gap is that much larger, though.
