Here's one possibility:

noMMR = \once \override MultiMeasureRest #'transparent = ##t
\new Staff << { \noMMR R1^\fermataMarkup } { a4 b c' d' } >>
> (In the actual case I want a tremolo under the fermata.)
In that case, you might want to define a single markup containing the tremolo
and fermata.
