Hoo boy. This is an exceptionally tricky problem. See also http://lsr.dsi.unimi.it/LSR/Item?id=326. This is my attempt. I am using version 2.17.95, but I think it should also work for yours. It does alter pitches and rhythms, so if you want good MIDI, you'll need to use tags as in the above snippet.
