There's quite a bit going on in this score.  The pinch point is in measure 9, 
where beamed quarter-notes switch staves.  I have both approaches tagged, and 
the same music is typeset using either approach.

The score is first typeset with a hidden raised note, which does the job.  Then 
it's typeset with a raised "XXX" markup.  The markup goes into the upper staff 
without affecting distance.
