Just a reminder, the Lincoln Council on Aging has the “FILE OF LIFE” packet which contains crucial medical information which you put on your refrigerator door. It is held in place by a magnet that attaches to the refrigerator door. It is the first thing EMT and fireman look at when they arrive at a home for an emergency, if the patient is alone or unable to speak. The form lists your full name, doctor’s name and contact information, health care insurance name, closest relative or friend, medications, allergies and a health care proxy . Diane Clapp
