This is strange as I have not encountered this issue before and most of my tests are 
done on Mac OS 9.2.2. Could it be that you have multiple versions of the Reader on 
your machine? If so, then I think that this may be the source of the problem. 

Please make sure that there is only one version of Acrobat or the Reader on this Mac.

There seems to be a problem with inm's pdf xtra and macOS 9.2.2.
Windows all versions + acroreader 5 works fine
macOS 9.1 + reader 5 is fine
