I may not fully understand what you are asking, but it sounds as though you are intending to use a SMUS, but are considering the issues as though you would not be, and would be making peer level connections. If you use the SMUS, each client does not need to know the IP address of the other clients, only that of the server, which becomes the "mailman." To reach beyond your router, assuming the server is also using a "local" IP, you would need to set up port forwarding on the router to the port that the SMUS is using on the server.
