Not going to go into setting it up for incoming calls b/c you have that working but for outgoing calls, you can either get an app that does a callback for you or you can use pbxes.org who have set there accounts/network which is free to be integrated with google voice, so when you dial a number or sip address in your linphone sip client, it will directly go through google network and make the call for you like you were in gmail and made the call using the gmail phone calling option. I use pbxes.org with sipdroid client on my phone and it works perfectly.
In terms of callback option, on my mytouch 3g on tmobile network, we have an app for free called google voice callback, that solves the outgoing calls problem, but ever since pbxes.org intergrated for there users who have a google voice account with google voice so they could make outgoing calls, i have been just using pbxes.org as my sip address on my phone to make and receive calls.
