I use printconf-tui to set up print queues on my RH 7.2 LPAR image, and
I can print to them from linux (using JetDirect) and from windows via
samba on the linux LPAR. I've had no luck with getting any kind of
cover-page/banner to show up, though.

From everything I've read with respect to lpr/LPRng, a banner page is
supposed to be produced.

my printcap started with a suppress headers line: ":sh:\" ... no value
is supposed to represent false. I copied the definitions to
printcap.local and tried explicitly stating ":sh=false:\" and also
taking the line out completely since the default is supposed to be
false. Restarted lpd, a cat of /etc/printcap showed that it had seen my
changes to printcap.local, yet no header/banner page was ever sent.
I've also tried specifying always banner (ab), short banner (sb),
header last (hl). I've been working with two different printers (a
ricoh and an hp).

As far as lpr goes, the only related options I see are for disabling the
banner (lp -o nobanner, lpr -h).

Playing with other options like providing a title -T"banner" hasn't
cause a banner to appear.

Can anyone try pointing me in the right direction?
Thank you,
Daniel

p.s. this was the generated printcap:
