I'm not going to be able to solve your problem, but block device 58 (3a hex) is reserved for lvm according to devices.txt (which comes with any kernel tree). If you run 'cat /proc/devices', you'll probably see lines that looks like:
Block devices: ... 58 lvm
If you hunted through /dev, you might find something with a major number of 58, and a minor number of 17.

i'm using the snapshot option of lvcreate to create an image of an oracle database. for quite awhile it has worked fine; however, since Oct 31 I get something like this in the log when the snapshot volumes are mounted:
Nov 10 17:40:01 s99lxd02 kernel: kjournald starting. Commit interval 5 seconds Nov 10 17:40:01 s99lxd02 kernel: Can't write to read-only device 3a:11 Nov 10 17:40:01 s99lxd02 kernel: EXT3-fs: mounted filesystem with ordered data mode.
I don't have anything with a major device number of 3a. Also the database has been corrupted since then. any ideas?
