Chris,

I'm not going to be able to solve your problem, but block device 58 (3a
hex) is reserved for lvm according to devices.txt (which comes with any
kernel tree).  If you run 'cat /proc/devices', you'll probably see lines
that looks like:

Block devices:
  ...
 58 lvm

If you hunted through /dev, you might find something with a major number
of 58, and a minor number of 17.

I don't have anything with a major device number of 3a.  Also the database
has been corrupted since then.  any ideas?
