Check to see if a link to qmail-inject is in /etc/smrsh. Many sendmail replacements will refuse to run at all if the link isn't there.
Second, is /bin/mail or /bin/mailx setgid mail? Free suggestion: these days, qmail is pretty quirky and getting further off in the weird zone by the month. I'd *really* suggest switching to postfix or exim if you don't like sendmail. Either Postfix or exim are much easier to configure and have a lot fewer weird "features".
