We have several customers running a similar service with 50K+ mailboxes,
and one with more than 2M mailboxes.

You need to design somewhat differently for this environment to get
efficient usage. Single monolithic servers will behave badly in this
environment; you want to do this in a more modular way. You also
probably want to reconsider some of the tool choices; qmail performs
poorly in this environment.

Fairly normal for a mail environment with any real volume.

Are you swapping to real disk or VDISK? Increasing the virtual storage
will likely make things worse.
