For those following the "unable to load from CD-ROM" issue, it turns out
that it was a code-level issue in the HMC (my HMC is version 2.9 but I
don't remember what the service level of it is). My local CE just put on
some "patches" this afternoon (which are probably Linux-based since the
HMC is now Linux-based) and the option to load from CD-ROM is now
available.

I believe it is looking for the suse.ins file which is in the root
directory of CD1.  If you're pointing to an install tree via FTP, it might
be here:
# cat /nfs/sles9x/sles9xsp3root/sles9/CD1/suse.ins
* SuSE Linux for zSeries Installation/Rescue System
boot/vmrdr.ikr 0x00000000
boot/initrd.siz 0x00010414
