On my SLES10 SP1 system, the module is in /lib/modules/2.6.16.54-0.2.3-default/kernel/drivers/s390/scsi/zfcp_hbaapi.ko which means it was installed as part of the kernel-default package. The presence of CONFIG_ZFCP_HBAAPI=m in /usr/src/linux/.config confirms that it is part of the kernel source. The san_disc program is in /usr/bin, and is part of the s390-tools-1.6.0 package.
