Are you creating your own Linux distribution?  If not, whatever distribution 
you're installing will already have everything you need.

Again, if you're installing an existing distribution, those three things will 
be all that you need, along with the actual packages to be installed.  The 
initrd has all the binaries for the shell, SSH, etc.

One more time, if you're installing an existing distribution, just use the .ins 
file they have created.  If you're building your own distribution, just use the 
.ins file from some other distribution.  Those addresses are not (particularly) 
distribution specific.
