If the HMC Is reporting this connectivity issue it has nothing to do with the OS. Your IOCP deck looks fine, so the hardware is configured properly on your end, unless you made a change to the IOCP and forgot to activate / POR that change.
Are the link lights on? Is the fiber known to be good?
