Dave, Thanks.
It looks like it happened while formatting/partitioning disks (dasd). However this second time around it actually completed and did not get the error again. I'm concerned that it happened and will it occur again while it is running in a production environment. We have reported the issue to IBM support along with the PSW. Will see what they say.
