We were able to resolve the problem. Initially our SAN admin believed it was a
bad component on the switch (some sort of adapter) and that was replaced. It
seemed to improve the connectivity but after a while we would still have the
LUN path disconnects.

device-mapper: multipath service-time: version 0.2.0 loaded
sd 0:0:0:7: [sdj] Medium access timeout failure. Offlining disk!
sd 0:0:0:7: [sdj] Result: hostbyte=DID_TIME_OUT driverbyte=DRIVER_OK
sd 0:0:0:7: [sdj] CDB: Read(10): 28 00 02 90 18 00 00 00 08 00
end_request: I/O error, dev sdj, sector 42997760

We ended up putting the chpid offline and then online and that cleared up the
problem. I would assume that it reset the channel in some fashion. So far so
good. Thanks for all your help!

Subject: Re: Having issues attaching to LUNs after Brocade mode change

Here is a Brocade procedure for changing interop mode:

Solution Guide: Migrating from McData Fabric or Open Fabric Mode (IM2 or
IM3) to Brocade Native Fabric Mode (IM0)

http://community.brocade.com/dtscp75322/attachments/dtscp75322/fibre/2767/1/Migrating+from+McData+Fabric+or+Open+Fabric+Mode+%2528IM2+or+IM3%2529+to+Brocade+Native+Fabric+Mode+%2528IM0%2529.pdf

No actions are required on the FCP channels because it includes disabling,
reenabling ports at the switch. This is enough to cause the channels to
refresh their addressing information. Part of this process is re-logging in.

Also, it clearly states that all zoning information will be lost. It has some
steps for backing it up and restoring it. I'm not surprised that Chris is
having problems. I hope you figure things out without too much more trouble!
