some use a big spreadsheet

some nothing at all and just look for free devices and space in the disk
subsystem to create a new LUN, then ask for more when they run out.

some manage the whole thing using home grown automation driven from config
files that live ... somewhere

some use a combination of all 3 since they split the tasks across 3
different teams. ( Storage / SAN / Hosts )
