I talked with our Unix group. There was RPM's that SUSE send to us. But they 
need to execute twice for the issue disappear and appears to happen expanding 
the DASD not the SAN storage LVM. SUSE still working for a final fix. This is a 
findings from one of our Unix guy.

"All of our servers share the same layout:  DASD disks (from mainframe storage) 
are used for our main root volume-group and the rest (Oracle filesystems) comes 
from the "Open world", that is our SAN storage system (IBM v7000 
storage-array).  We have 4 paths for each LUN and all of them are consolidated 
via multipath. This multipath device is what we use as PVs for our 
volume-groups.

I was curious if our bug only triggers when we expand logical volumes backed by 
DASD devices... so we did a test now.  I asked for our test system to be 
restored and I performed the LVM/filesystem expansion with one of Oracle 
filesytem (/temp) that is backed by LUNs coming from our SAN.  To my surprise, 
the errors didn't show up !    Then I proceeded to expand /opt (from DASD) and 
indeed we get the error:"

Do you know what package was patched and version?  We tried a couple of patches 
that did not work but they were not as current as the one you are using.

We open an issue with SUSE, is a LVM issue. They sent a fix last Friday, it 
looks that is working. The Unix group are testing right now.

After various testing we found that the issue making the system doesn't boot is 
when expanding the LVM OS file system, ex. /opt, /var. The Unix administrator 
send a photo of a message when he resized the filesystem.

When he reboots the system, the system stop under Booting default (grub2) for a 
couple of minutes and then show the Dracut message going to Give root password

Right now there an open case under SUSE waiting for a solution.

you aren't the only one.

Something's definitely different.  Open a case with your Linux support.

For all least all of the system volumes (ones needed to boot) , consider 
erasing the 51-dasd* rules in /etc/udev/rules.d and then using "chzdev -e " to 
put back new 41-dasd-eckd* rules.
Then running mkinitrd
Then running grub2-mkconfig -o /boot/grub2/grub.cfg; grub2-install

These things or some combo of them have solved it when we hit it (after using 
rescue system get to a point where we could do this)

You can also "cat /run/initramfs/rdsosreport.txt" to get more info.  It will 
like complain of some missing devices or maybe  you simply have to vgscan or 
vgchange -ay that rootvg

Or boot with parm rd.debug as Mark Post mentioned here last week.

patch every Linux guest on a monthly basis. A server patch various month 
without a problem and for some unexplained reason after a successful
