You're posting a lot of information, but I'm going to ask some stupid questions anyway - just trying not to jump to conclusions before getting a crystal-clear picture of your setup.

Are you using JACK here, or directly ALSA? In other words, are you connecting 'in' to 'out' in the qjackctl 'MIDI' tab or in the 'ALSA' tab?
