1, 2, and 4 are the most common. It's common to only mic 1 of the cones, pretty close up - but it's also common
to do any bizarre combination of mics and placements you can

Some people also like to stick a mic right between the two cones of a 2-driver amp.
Miking an open cab from behind is also a popular trick - it yields a completely different
tone.
