Have you check if there is an IRQ conflict? "cat /proc/interrupts" will show
you the ones that are used. Is the (working) SB on the same interrupt as the
GUS? If not, try moving the GUS to the same IRQ.

I haven't tried Q3T (I'm on a P133 with no 3d accel so...) but I know that I
had problems with Quake2 before - I did get sound, but it would turn off and on
all the time (I've got a GUS to - GUS MAX). Installing ALSA (Advanced Linux
Sound Architecture - alsa.jcu.cz (I think)) fixed the problem.
