Are you using diald with pppd?  Diald handles most of the setup of
pppd.  The /etc/ppp/options file is empty in many cases.  I only have
to use the pap-secrets and chap-secrets.  All other pppd configuration
can be done through diald.conf.

Have you tried manually dialing up with e.g., minicom and then kicking
off pppd by hand.  This always lets me see exactly what is happening. 
Then make sure whatever you did is in the scripts that you use.

You mention that you are expecting a login prompt.  You may not get it.
My ISP does not provide a login or any other output.  It sits there
waiting for me to start sending the PPP packets.
