I've had the same problem too. It turned out that my modem didn't reset
after a hangup. I think it has to do something with the way pppd or diald
hangs up: by dropping the DTE (?) signal, while the modem might expect a
ATH0 command.
I added a reset command (ATZ) to the chat-ppp (connect) script, just
before the modem initialization string. Now everything works fine.
