2.6.22 had a fix for the 7021a, with respect to DiSEqC. Do you have a vp7021 (starbox) or vp7021a (starbox II)? On my starbox II, 'usb in operation failed (-110)' generally means tune failed to lock. This could be because of a failed DiSEqC command, attempting to tune something that isn't actually there, or many other problems.
