Did you try without the AGP card? If the PCI card works, then the problem is that the Apple ROM does not initialize all cards, but only one of them. Since the fb driver does not know how to initialize the card, it is left in an undefined state.
Hmm. If all else fails, I'll try to play around with the hardware (remove the card, etc.). I haven't opened this machine in 2 years, it's, er, not very convenient to pull it out of where it is at the moment...
It's hard for me to believe the card is not initialized, as multihead works out of the box in MacOS. If the card wasn't initialized, would it be recognized by Linux at all? /proc/fb does see the second card (reporting it as Mach64). Besides, another distro I tried (yellow dog) recognized the second card, not the first one.
Supposing that it isn't initialized, is there a way to initialize it from Linux?
