I'd like to echo Ira's sentiments and say that easiest would probably be
to remove sendmail and put something else instead - I recommend postfix.
It should be a minimal effort to reconfigure it to forward email like
sendmail (its 2 lines in main.cf IIRC). After you do that, plugging in
something to filter spam is relatively easy.
