Thanks - but the book it mentions is circa 1999, Apache 2 alpha was release
at 2000 with final release at 2002. I don't suppose this book covers that,
does it?

That's  what I expect to be the authoritative source but as I mentioned in
my original question it isn't up to date. The "Apache API Notes" page was
last touched on October 2003 (from the HTTP headers). Do you have any idea
which version it tries to cover and whether this information is relevant for
2.2?

Thanks for the highlight. I didn't notice the recommendation for the mailing
