Last time I had something like this, as far as I remember, I had to
make sure I "losetup -d ..." the device (I was playing around with Xen
DomU's disk images from the Dom0 using losetup, xpartx and vgchange).
What does "losetup -a" shows?
Also maybe worth checking output of "lsof".
