Begin at the beginning - by implementing the patch for qpopper (or whatever
pop3 server you are running).  I think newer versions have an option for
configuring pop before smtp without hacking source code.  Similarly Sendmail
8.10 fully supports SMTP AUTH for authentication before relaying mails.
Implement that as well.
