Use pnpdump to generate the output: 

> pnpdump > /etc/isapnp.conf

Edit this file, and since you know the irq and io address, it is simple.

See man pnpdump for details.  You need to then add appropriate entries in
the conf.modules or modules.conf depending on the distro you are using.
