It actually happened more than once .... and on production boxen at that.
One of my friends was at Satyam about five years back - got root on an old unix
box with c shell the default.  He wanted to clear out all the files in his /tmp
- and forgot to see the pwd, and what he was logged in as.  # rm -rf / tmp
  works quite well ...
