Most mail delivery/user software create a non-existent mailbox if a mail is to
be stored in one. The deletion of empty mailboxes in mutt is configurable via
set delete=no
in your .muttrc.

Mutt just invokes the mail transport agent that you have specified in your
.muttrc - defaults to
set sendmail="/usr/sbin/sendmail -oem -oi"
It is sendmail (in this case) that decides whether to queue or immediately
send the mail, not mutt. And sendmail stores queued mails in
/var/spool/mqueue.

There are many mail retrieval software available, but the most popular one is
as you say, fetchmail. Mutt can also access a pop/imap account, newer mutt
versions understand an invocation as mutt -f imap://imap.server as a command
to open an imap folder.
Where new mail is stored? Depends on your system configuration, but all linux
distributions I have seen defaults to /var/spool/mail/<username>

You needn't tell mutt about the default inbox, it knows about it. And by
outbox, if you mean where to save mails for later sending, then put
set postponed=/path/to/postponed/mailbox
works.

PS: You shouldn't be using the root account for mail etc., add a normal user
account and work from there.
