Might be a network connectivity problem.  Try using a local smtp server
(eth.net or whatever your ISP is) as the smarthost.  Then turn off dns
lookups at your side, etc - see http://www.hserus.net/sendmail.html (all this
is to minimize delays due to dns and such at your side - but if it is a
network connectivity issue it might be hard to debug)

One reason can be because you need to adjust the MTU / MRU on your modem
settings - see the sendmail FAQ at http://www.sendmail.org/faq/ for more.
