This might be a problem related to qmail blocking mail relay. If you have ucspi-tcp installed, try this:
In /etc create a file called tcp.rules with the following.
127.:allow,RELAYCLIENT=""
<network address from which relaying is allowed>.:allow,RELAYCLIENT=""
Now execute this: tcprules tcp.smtp.cdb tcp.smtp.tmp < tcp.rules
Now check that you are including -x /etc/tcp.smtp.cdb as an argument while you're invoking qmail-smtpd.
