It is (a), i.e "nn" hours after the user has logged in.

Hereis the explanation.

     "authenticate_ip_ttl"  is used to deny users until ' ttl time ' has
been expired , who sharing their password between them .

        e.g, Assume  "authenticate_ip_ttl 360 seconds" , and user Mega
surfing the net by specifying her pasword .That time if another user Dhana ,
tries to surf using Mega's password means, Dhana will be denied  until the
specified time . So you can specify any time , what you need .

In other words,
Essentially,  the first parameter authenticate_ip_ttl   requires a "time
duration" specification.   Let us say the time duration specified is T
seconds.  This is the duration for which any user login will be bound to a
specific IP address, i.e. once a user U1 has logged in from IP address ip1,
then none else can log in as user U1 from a different IP address for the
next T seconds.  If someone does attempt to log in as that user U1,  then
both user logins are denied (even the first user U1 will be
reauthenticated).

Hello,

The "authenticate_ip_ttl  nn hour" tag in the squid (2.5) configuration
file remembers the IP address associated with a user for "nn" hours.
But does than mean
a. "nn" hours after the user has logged in
(or)
b. "nn" hours after the last request from the user?

The squid documentation is not explicit in stating the difference:
"If you use proxy authentication and the 'max_user_ip' ACL, this
directive controls how long Squid remembers the IP addresses associated
with each user."
