This is most likely because the owner/group of .fetchmailrc and the
UID/GID of the fetchmail process are different.

/etc/ppp/ip-up is run with UID/GID of the ppp process (most likely 0/0).
chown root:root .fetchmailrc; chmod 600 .fetchmailrc should do it.
