Need more details on your setup to be able to help you.

- What is the config of your asterisk server? How many simultaneous calls?

- What are the two endpoints in the picture?
one of them is a snom 200, where and what is the other end? Is it a user on an analog phone line (through a zaptel device) or another SIP / IAX user? If that user on the same network or is this happening across the internet?

- If there is a zaptel device in the picture, try running zttest and look for missed IRQs.

- If the call is going over the internet, check for packet loss and latency.
