Thanks :) Now I found the somewhat alarming algorithm in section 4.9.2. This will be a bit unpleasant, though, since the _PAGE_CACHE_xxx macros will become rather confused. I suppose there's no fundamental reason that pgprot_t has to correspond to pmd bit positions.
