Most of the actions described by access_db are carried out during message reception.
When you use smap, *it* is responsible for collecting the message and any action
to filter messages (like anti-relay measures) must be implemented by smap. In this
scenario, sendmail enters in the picture too late to do anything useful in that
respect.
I suggest browsing the FAQ at www.fwtk.org that will point you at several patch sets
that will let you control what to accept and what to reject at the smap stage.
