The adapter is from SIMPLE TECHNOLOGY. I bought it from 
PC Connection, Inc. www.pcconnection.com/

It's called a Smart Media Card Adapter, and from a sticker
on the back of the type II PCMCIA card, I can see what 
appears to be a Manufacturer's part no. 90-104-24211,
and on my invoice I also have the number STI-SMAD listed
next to the Adapter. I bought it for $59.95 last Jan. 6th.

The vendor part number on my invoice is 51149; I suggest 
going to pcconnection.com and doing a search for 51149.
