I had been using Cat5e. It appears that could be the culprit. Switching to Cat6 
fixed my problem.

Curious: were they all Cat6 cables?  Gig calls for Cat6, but Cat5 will
work, hit or miss.  If they're Cat5, you just stumbled onto one that was
"just good enough".
