Can I get some help on what replacement controller to buy?

I have a PE 2800 with a RAID-5 with three seagate 36gig drives on a PERC 4e/Di. 
   Currently all drives are ONLINE and working.  But the server has been up 
since 2004 and has crashed three times in recent days with various error

Upon reboot the file systems check-out file (thanks to the journalling).

Some key things about the controller are:
# megamgr command tells me.....

Fw Ver = 5130
Bios Ver = H418
Backup Module = PRESENT
Battery Pack  = PRESENT
Temperature   = GOOD
Voltage       = GOOD
Fast Charging = COMPLETED
No of Cycles  = 1

If the hard drives are not going to OFFLINE status, then could my controller be 
going bad?   Is this an onboard controller?  I want to avoid replacing the 
motherboard if I can.  Can I disable the onboard controller, get a regular PERC 
4 controller add-in card which will inherit the existing RAID 5 array 
configuration while preserving the data on the hard disks.  I'm really hoping 
to not have the re-install the OS since a full system backup may not be 
possible if this onboard controller is acting up.
